Hepaticojejunostomy is a reconstructive operation that connects the hepatic bile duct to the jejunum to restore bile flow.
When is it considered?
It can be used to reconstruct bile flow after complex bile duct injury, benign strictures, excision of biliary cysts or selected bile duct cancer operations.
Preoperative planning
Planning considers biliary anatomy, vascular relationships, infection and any existing drains. Timing after bile duct injury and the degree of inflammation are important.
General principle of the operation
A Roux-en-Y limb of jejunum is typically brought to the bile duct and a precise biliary-enteric anastomosis is created.
General postoperative course
Postoperative follow-up monitors for bile leakage, infection and later anastomotic stricture.
Possible risks and complications
Potential complications include bile leakage, intra-abdominal infection, bleeding and later stricture or cholangitis.
