Bile duct injuries are serious biliary complications most often recognized during or after gallbladder surgery.
What is this condition?
Injury ranges from a small bile leak to major duct transection or tissue loss; benign strictures can develop later.
Signs and symptoms
Postoperative pain, fever, bile leakage, jaundice or fluid collections may occur; strictures may cause recurrent cholangitis.
Diagnostic assessment
Accurate definition of biliary anatomy is critical. MRCP, ERCP or percutaneous cholangiography may be used selectively.
General treatment framework
Endoscopic stenting, percutaneous drainage or surgical reconstruction are selected according to injury type and timing.
Role of surgery
Roux-en-Y hepaticojejunostomy is a commonly used reconstruction for complex major bile duct injuries.
